Summary

Responsible for providing medication, treatment, support and rehabilitation services to all residents.

Sunday 6:00am-2:30pm

Monday 6:00am-2:30pm

Tuesday 6:00am-2:30pm

Thursday 6:00am-2:30pm

Saturday 6:00am-2:30pm

Sunday 1:00pm-9:30pm 

Essential Duties

  • Processes medication orders by physician, ensuring all required documentation is complete.
  • Maintains/controls assigned medication supplies; coordinates prescriptions and delivery, storage and destruction of medications and monitors and maintains medication records in accordance with Agency policies, FDA requirements, and contracts
  • Completes medical assessments as needed; records history and vital signs; determines whether resident is under the care of a physician; and gathers related information.
  • Monitors and oversees patient care to ensure that care plans and physician’s orders are being followed. Documents changes in condition and contacts physician or family members as required and follows up on health care as prescribed by physician/nurse (example ensuring lab tests are completed).
  • Accurately administers and documents medication and treatments. The medications include oral meds as well as injections and topical medications (lotions, creams, and so forth).
  • Monitors residents for concerns relative to psychotropic and non-psychotropic medication side effects and compliancy; discusses side effects with prescribing physician when necessary to determine if changes should be made.
  • Educates and communicates with residents on the potential benefits and side effects of medication, working with residents to develop their awareness and understanding of their symptoms.
  • Provides families and residents with psychoeducation in the acquisition of independent living skills; (i.e. proper administration and storage of medications, medication side effects, seeking out and utilizing medical community resources).
  • Ensures complete and accurate documentation of services provided to resident for contract compliance, statistical and financial reporting and other purposes as necessary.
  • Supervises or arranges supervision of sick children when needed and ensures that proper medical care is administered in accordance with physician orders or routine medical expectations.
  • Assesses risk factors that may threaten children’ safety and responds appropriately and within guidelines should risk escalate or abuse occur.
  • Provides safe, punctual transportation for resident children to attend medical, dental and other similar appointments, which can include emergent, medically related, or other as needed visits or transportation.
  • Qualifications

    • Completed an accredited nursing program and maintain an active LVN license from the State of California.
    • Previous employment experience with special needs children preferred.
    • Effective communication skills both oral and written.
    • Ability to be flexible and work in a changing environment.
    • Ability to pass and utilize the Pro Act physical restraint system.
    • Ability to work as a member of a therapeutic team.
    • Computer skills sufficient to meet production demands of the job including Microsoft Office and WELLIGENT software for program notes.
    • Ability to work varied hours including evenings, nights and weekends.
    • Sensitivity to the different cultures represented among the residents, families and staff.
    • A basic understanding of age-appropriate behavior for children/adolescents.
    • Knowledge of emergency procedures of the Agency.
    • Ability to model positive behaviors and Agency values.
    • Skill in maintaining detailed, accurate and complete records.

    Compensation Range: $26.61/hr - $35.48/hr
